You are here because you've been granted access to your company's Edge application. We've found that not all users are ready to dive in with how to use a new platform on day one, though, so we've created this quick primer to help get started.
Post-Acute Care
Following a hospitalization for injury or illness, many patients require continued medical care, either at home or in a specialized facility. Post-acute care refers to a range of medical care services that support the individual’s continued recovery from illness, management of a chronic illness or disability, or transitional end-of-life care.
Post-acute care includes services that beneficiaries receive after, or in some cases instead of, a stay in an acute care hospital. Depending on the intensity of care the patient requires, treatment may include a stay in a facility, ongoing outpatient therapy, or care provided at home.
Though Trella Health solutions support a variety of clients who cover additional services across the post-acute spectrum, the Edge platform focuses on three pillars of care: HME, DME & Infusion services.
Home Medical Equipment (HME) & Durable Medical Equipment (DME) Agencies
Items that are essential for supporting a medical need have been referred to as durable medical equipment (DME). These include ambulatory aids, special hospital beds, oxygen tanks, wheelchairs, special commodes, compressors and more. HME services are associated with DME, but focus on the skills and services required, such as: delivery, assessment, insurance facilitation, and more!
Infusion Agencies
Infusion therapy refers to the process in which patients receive medications via needles or catheters in a vein. It is prescribed when a patient’s condition is too severe to be treated effectively by oral medications. Receiving this care at home rather than a healthcare facility or other care setting is a more cost-effective option, and is referred to as “home infusion therapy.”
Collectively, these are called Business Lines in Edge. You may find many places within the different modules or reports that refer to business lines, depending on what your company has purchased, but more on that later!
